Notice of Data Event

What Happened? MVP VIP Holdco dba Heart of America Eye Care (“HOA”) has been actively investigating a cyber security event in which an unauthorized third party gained access to certain files within our network. The investigation determined that certain information was viewed or taken without authorization between April 1, 2026 and April 7, 2026. We are currently conducting a review of the relevant data to determine if Protected Health Information (“PHI”) is involved.
What Information Was Involved? The affected information is currently being reviewed. We will notify any affected individuals directly at the conclusion of that review.
What We Are Doing. We take this event and the security of personal information in our care seriously. Upon becoming aware of the event, we took steps to secure the network, investigate the activity, and determine the scope of impacted information. We engaged third party specialists to investigate and respond to this event.
What You Can Do. We encourage you to remain vigilant against incidents of identity theft and fraud by reviewing your account and free credit reports for suspicious activity and to detect errors. Suspicious activity should be promptly reported to relevant parties including an insurance company, healthcare provider, and/or financial institution.
